Minas Tirith means Tower of the Guard. During the Lord of the Rings story, the line of kings had been broken for about a thousand years, and Stewards ruled in the absence of a king. The city was built on seven levels, with a citadel at the top. The Crown of Gondor has seven jewels of adamant, represented here by seven stars, which is also the symbol of Elendil.
The fortress of Helm’s Deep was named after Helm Hammerhand, after fighting against the Dunlendings there.

“Helm was known as Hammerhand because of his great strength; he killed [his enemy] with one blow of his fist.”

Osgiliath was the Old capital city of Gondor.

The Dome of Stars, which housed a Palantíri (one of the Seeing Stones, and represented in this design by an eye in a circle), was atop a great stone bridge which connected the city since it straddled the Anduin River.

Osgiliath sat about halfway between Minas Anor (Tower of the sun) to the southwest and Minas Ithil (Tower of the Moon) to be northeast, both of these are represented by sun and moon symbols in their respective positions

Amon Hen, where the Fellowship breaks. Frodo and Aragorn both sat upon the Seat of Seeing at this place.
